Setting the Course: What Is Dry Eye?
Before we dive into the depths, let’s consider our charts and define our terms:
Tear Film: Picture this as the shimmering surface of the eye—an ultra-thin layer of liquid that keeps our eyes moist, nourished, and ready for adventure.
Ocular Surface: Think of this as the coastline—the land where the tear film meets the eye. It includes the cornea (the clear front window), the conjunctiva (the covering over the white of the eye and the inside of the eyelids), and the eyelids. A delicate ecosystem which should exist in balance.
Assessment
When should we think of dry eye? When our eyes may feel watery, burny, gritty or itchy, perhaps even like some landlubber kicked sand in your eyes on the beach. Our vision may be blurry and our eyes may appear red. We must properly assess the situation so we can plot our best course possible:
Education: Let's enlighten our fellow travelers about dry eye — its causes, symptoms, and potential impact on vision. Knowledge is our compass.
Local Environment Modification: Simple changes can help create a positive environment, we consider factors like humidity, wind (okay, maybe not wind), and screen time.!
Our Medications: Some medications can be like rogue waves, causing dry eye. Considering these can allow for best care and, at times, working with our fellow medical colleagues to make adjustments.
Lubricants: Preservative free artificial lubricants can be our lifeboats — keep the tear film shipshape. If the meibomian glands (those little oil factories) misbehave, we bring out the lipid-containing supplements.
Lid Hygiene: Scrub-a-dub-dub! Warm compresses and lid massages keep the eyelids tidy. This really should be part of your routine, like brushing our teeth every day.
Plotting the Journey: Staged Approach
Level 1 - Calm Waters: For mild dry eye, we recommend non-preserved artificial tears. They help calm down some of the early stages of dry eye disease.
Level 2 - A Bit Choppy: When the seas get rough, we fend off boarders like those pesky Demodex critters, medical eye drops to help calm the inflammation, and moisture chamber spectacles (fancy eye goggles).
Level 3 - Full-On Tempest: It's time to look further into our options of in office treatments, specially formulated eye drops and therapeutic contact lenses (the eye equivalent of a cozy cabin).

Tempsure Radio Frequency: Precise heat therapy using radio wave energy, helps restore meibomian gland production and unblock the glands. Bliss! A nice side benefit is the rejuvenation of skin elasticity.
Intense Pulsed Light (IPL): The healing power of light reduces inflammation in and around the eyes! It works wonders!
Topical Medications: Steroids, cyclosporine, and lifitegrast are all useful medications that work directly in the affected areas.
At-Home Care:
Low Level Light Therapy (LLLT): Calming light therapy, the latest addition to in office treatments in the dry eye treatment spectrum. LLLT has been long-established to help promote healing and reduce inflammation.
Omega-3 Supplements: Fish oil for your eyes? Sounds fishy, but it works! We will guide you to the right combination of the right kind of oils, so you're not all lost at sea.
Dietary Modifications: Skip the salty pretzels; let's review the charts and plot a course to overall wellness through great nutrition. Don't worry, there's no hard tack here! We'll set you on the right course!
Warm and Cold Compresses: Dedicated use of heat and cold therapy to either promote tear oil production or calm the symptoms of inflammation!
